The ASHP Practitioner Recognition Program is intended to recognize excellence in pharmacy practice and promote public awareness of pharmacists who have distinguished themselves in practice.

Call for Nominations
Candidates for ASHP’s Practitioner Recognition Program must be members of ASHP and must have:
- Demonstrated sustained practice excellence in health-system pharmacy for at least 10 years,
- Contributed to the total body of knowledge in pharmacy practice,
- Demonstrated ongoing commitment to educating practitioners, and
- Worked to support the profession (e.g., through active membership and participation in ASHP).
Nominating Instructions
Applications should be sent to Director, Affiliate Relations Division, ASHP, 7272 Wisconsin Ave., Bethesda, MD 20814. The annual deadline is October 1.
Selection Committee
The ASHP Commission on Credentialing administers the Practitioner Recognition Program and chooses awardees after due consideration of all applications. Pharmacists designated as Fellows of the American Society of Health-System Pharmacists (FASHP) are recognized at ASHP’s Summer Meeting.
