ASHP Drug Product Bar-Code Problem Reporting Center
Report a bar-code medication problem to ASHP if one of the following scenarios is identified:
- Bar Code not readable by scanner
- No bar code on UD product
- Scanned, but reads as another product
- Other Bar Code problems that interrupt patient care or pharmacy efficiency
In order for the ASHP to better serve its members needs with Bar Code medication initiatives:
- It is vital for members and others responsible for managing pharmacy information systems and pharmacy inventories to report their bar code medication problems to ASHP.
- Reports assist ASHP in learning about and assessing the extent of bar code medication issues.
- Aggregate information may be shared with the Food and Drug Administration, PhARMA, GphA, and other manufacturers to advocate for the inclusion of usable bar codes to improve patient safety.
ASHP depends on members for information to assist in making decisions, setting priorities, and planning and implementing actions to address medication problems.
