Toby Clark, R.Ph., M.Sc., FASHP, is a Medication Systems Consultant and Clinical Associate Professor of Pharmacy Practice at South Carolina College of Pharmacy. He also is a consultant to the Director of Pharmacy at the Medical University of South Carolina in Charleston. In addition, Clark serves as a Lead Surveyor for the Accreditation Services Division of ASHP. Previously he has served as (Adjunct) Professor of Pharmacy Practice and Director of Hospital Pharmacy Services (retired) at the University of Illinois at Chicago Medical Center and College of Pharmacy.
Clark is a member of the American Society of Health-System Pharmacists (since 1967), South Carolina Society of Health-System Pharmacists, American Institute of the History of Pharmacy, American Medical Informatics Association, and is a past officer of the Southeastern Michigan Society of Hospital Pharmacists and the Society of Hospital Pharmacists of Greater Kansas City. He is a past North American Vice President and current Treasurer of the Section of Hospital Pharmacy of the International Pharmacy Federation (FIP). An avid medical/pharmacy historian, Clark is Treasurer of the Robert Wilson Medical History Society of Charleston. In 2003, he was awarded the Donald E. Francke Medal for his contributions to international pharmacy.
Clark has served two terms as Chair of the ASHP Practice Management Group and has participated as Delegate to the ASHP House of Delegates for the states of Texas, Missouri, and Michigan. In 1998, he was recognized as a Fellow of ASHP. Clark is a Past Chair of the University Health System Consortium Pharmacy Council and is a founding member of both the Oncology Section of the Board of Pharmaceutical Specialties and the Novation Pharmacy Executive Committee.
Clark was born in Indiana and received his B.S. degree in Pharmacy from Ohio Northern University in Ada. He completed an ASHP-accredited general residency from Bronson Methodist Hospital in Kalamazoo, Michigan, and a M.Sc. degree in Clinical Pharmacy from Wayne State University in 1970. He has held positions as Director of Pharmacy and faculty member at the University of Missouri at Kansas City as well as with several pharmacy management corporations.
Clarkâs professional interests are focused in the safety, quality, and productivity of the medication system, training pharmacy residents and in the rational use of informatics and technology. A frequent lecturer on topics of pharmacy/medication system informatics and computerized prescriber order entry, he spearheaded formation of the group POE CPOE list serve, now operated by ASHP. Clark has been a consultant to scores of hospitals in the United States. He strongly advocates hospital pharmacists serving patients through the safety and quality of the medication system.
