Please follow the steps below to apply for the 2011-12 Student Leadership Award. To be accepted, all application materials (mailed and electronic) must be received by ASHP no later than 5:00 pm EST on January 30, 2012.
Step 1: Complete paper application form [PDF].
Step 2: Submit all application materials (including paper application form signed by faculty member, Professional Activities Form, your CV and/or resume, essay, and 3 signed and sealed letters of recommendation). Make sure your application in typed, printed one-sided, and not stapled. Mail to:

2010-2011 Award Recipients
Class of 2011
Alexander H. Flannery
University of Kentucky College of Pharmacy
Mr. Flannery has taken on numerous leadership roles on the local and national level. While serving as the Chairman of the Kentucky Alliance of Pharmacy Students, he also served on the ASHP Student Society Development Advisory Group. At Kentucky, Mr. Flannery has received numerous awards for leadership and academic achievement. His health-system pharmacy achievements include a VA Learning Opportunities Residency Student position and an internship at a large teaching institution.

Tiffany K. Pon
Purdue University College of Pharmacy
Ms. Pon has served in various leadership roles including President of the Purdue Student Society of Health-System Pharmacy, and on the national level on the ASHP Student Society Development Advisory Group. Outstanding achievements also include being appointed as members of Phi Lambda Sigma and Rho Chi. Her health-system pharmacy experiences include a current role as a medication reconciliation technician for a major teaching institution and career interests in emergency medicine and cardiology.

Jennifer J. Cerdena
University of Utah College of Pharmacy
Ms. Cerdena's achievements include serving as the University of Utah Student Society president and leading a major initiative involving medication take-back events in the Salt Lake City area. She has also demonstrated health-system pharmacy interest as an intern at a major health system and has received numerous academic awards at the College of Pharmacy.

Charles T. Makowski
Wayne State University
Mr. Makowski helped found and develop a successful student society at Wayne State University and currently serves in leadership roles within the society and the Wayne State University Pharmacy Alumni Affiliate. He has been involved in mentoring and service in the school of pharmacy. He has also gained experience in health-system pharmacy through internships in different areas at a major teaching institution and through research in the transplant area.

Karen Craddick
University of Washington
Ms. Craddick has served as a leader in her local health system by mentoring and directing interns and revising the pharmacy intern program. She has been a leader in her student associations and School of Pharmacy class. She has also achieved academic success and has been recognized by the NCAA for her academic and athletic achievements.

Dazhi Liu
The University of Iowa
Ms. Liu has had a unique opportunity to serve health-system pharmacy by working as an education and training coordinator for mobile health clinics and focusing on pediatrics in an internship at a major teaching institution. She has also served as a leader both within her student society of health-system pharmacy and the Student National Pharmaceutical Association at the University of Iowa. She has excelled academically with numerous presentations and publications.

Heather Woodward
University of Colorado
Ms. Woodward has served in many leadership roles in the Colorado Student Society and her local Phil Lambda Sigma chapter. Her health-system pharmacy achievements include working as a clinical pharmacy intern for a large integrated health system where she focused on hospital admissions due to adverse drug events and other clinical projects. She has also received numerous scholarships and awards for her leadership and academic achievements.

Meenakshi Girish Shelat
University of Michigan College of Pharmacy
Ms. Shelat has served as a leader in health-system pharmacy by serving on two ASHP Student Forum Advisory Groups and on the local level as president-elect of her student society at the University of Michigan. She has also been involved in research clinical trials and serving as an intern focused on investigational drug services.

Nola Fry
Texas A&M Rangel College of Pharmacy
Ms. Fry is a leader at the Texas A&M student society and on the state level at TSHP. She has also focused her leadership and volunteer work on providing health care to the under-served populations in her state. Her health-system pharmacy achievements have included creating an antibiogram for a specialty hospital and serving as an intern at a community hospital.

Christopher I. Lai Hipp
University of Hawaii-Hilo College of Pharmacy
Mr. Lai Hipp has served as a founding member of the student society at the new pharmacy school in Hawaii. He has been involved in health-system pharmacy practice as a clinical pharmacy intern at a large integrated health system. He is also achieving academic success in pursuing his Pharm.D. and masters degree in Tropical Medicine & Pharmacology.
