The SSHP may submit at any time, but no later than June 30, 2012, the following items to be considered for 2012-2013 ASHP Recognition. (Please submit all electronic documents in one transmission.) Applicants must utilize template documents for items 4-6.
- Paper application form completed and signed by faculty liaison/advisor

- Cover letter explaining the organization’s mission and specifically, how the organization promotes the development of health-system practitioners. Please also describe your organizational structure including any routine committees, sections, interest groups, etc.
- One copy of any existing organizational documents (i.e.: charter, bylaws, constitution, or other documents that describe the structure and mission of the organization)
- Member roster

SSHPs that complete the activities required for recognition and submit completed items above by the June 30, 2012 deadline, will receive ASHP Recognition for the 2012-2013 academic year.
