Requirements for recognition (regarding membership, career development, and the professional development project) must be met each year to receive yearly recognition.  For continued recognition, submission of the following items are required by no later than the published deadline of June 30.

• Electronic Application Form
• Completed and signed paper application form [DOC]
• Member roster in excel format [XLS]
• Officer roster in excel format [XLS]
• Description of completed professional development project [DOC]
