25 mg/mL, 5 mL vial (NDC 55390-0465-05), Bedford
25 mg/mL, 5 mL vial (NDC 00703-5003-01), Teva/Sicor - discontinued
Reason for the Shortage
- Apotex discontinued their fluphenazine for unknown reasons.
- Bedford’s product is not available. The company has temporarily suspended manufacturing and distribution of all products while investigating a potential cross-contamination issue. Bedford estimates the suspension may be lifted by the end of July, 2009, but cannot estimate when product will be available.
- Teva/Sicor discontinued its fluphenazine decanoate product in 2007 for unknown reasons.
Estimated Resupply Dates
- APP has their 25 mg/mL 5 mL vial (NDC 63323-0272-05) presentation available.
- Bedford has its product on back order and cannot estimate a release date.
- Supplies of fluphenazine hydrochloride injection are not affected by this shortage.
