Medication Reconciliation and improving the transitions of care are recognized critical steps in the management of patients. The Joint Commission reports that the most frequently found root cause in reported sentinel events is the break down of communication. While pharmacists have taken a key leadership role in successfully implementing the medication reconciliation process, many continue to struggle with the challenges of meeting the National Patient Safety Goal.
