The SAG on Ambulatory Care Informatics will develop resources for members on electronic prescribing (ePrescribing), personal health records (PHRs), medication reconciliation, and electronic reimbursement issues (MTM-Clinical services documentation and billing for medications).

Key Information

Charge

  • Share information and provide guidance to improve e-Prescribing, especially:
    • Status and next steps for characterizing, defining requirements and adopting Standardized SIGs
    • Status and advocacy for full implementation of the eRx discontinuation message intended to go to pharmacies
    • Implementation of controlled substance e-Prescribing
    • e-RX Drug-Drug Interaction Alerting
  • Directly support the work of the Pharmacy e-Health Collaborative through information gathering and sharing
  • Influence HIT standard developing organizations (e.g., HL7) and certification bodies (e.g., CCHIT) to include the practice needs of the pharmacist in their requirements and criteria to achieve the safe, effective use of medications
  • Identify strategies and best practices for pharmacist online documentation of pharmaceutical care, especially:
    • Information required for successful pharmacist handoffs between providers and venues
    • Medication History Taking & Medication Reconciliation documentation
    • Medication Therapy Management (MTM) tools and MTM documentation
    • Online documentation intended to enable a pharmacy practice suited to Accountable Care Organizations
