The ASHP Pharmacy Practice Model Initiative (PPMI) can be a meaningful driver of practice advancement in U.S. hospitals and health systems.  To help our state affiliates tip the balance for practice change, ASHP has introduced an implementation tool kit for the ASHP—ASHP Foundation Hospital Self Assessment, created in collaboration with the Pharmacy Society of Wisconsin. 

 

The Hospital Self Assessment is a new web-based assessment tool to help pharmacists determine how their hospital or health system aligns with PPMI recommendations.

 

The tool kit provides guidance, resources, and turn-key materials affiliates can use to initiate state-level PPMI activities, including:

 

- A suggested plan for dissemination of PPMI to affiliate members,
- Agendas for initial leadership meetings,
- Outlines and agendas of meetings for educational program,
- Sample presentations, and
- Sample email communications.
