By submitting this form you ("you", "your", or "I") hereby authorize American Society of Health-System Pharmacists ("ASHP"), and its officers, directors, licensees, successors, sponsors, employees, and assigns (collectively, the "ASHP Representatives") to use, edit, republish, reproduce, exhibit, broadcast, advertise, and exploit any and all of the material submitted to ASHP, including, but not limited to any photograph(s), video, audio and/or interview information (hereafter, collectively "Materials") for the unrestricted permission, right and license to use Materials in any of its products or services, including but not limited to publications, presentations, web sites, social media, advertising, or in any other media chosen by ASHP. You also waive any right to inspect or approve any Materials that may be used in conjunction therewith or to the eventual use that any Materials might be applied to now or in the future.
You hereby agree and accept that the Materials may be released by ASHP and/or ASHP Representatives to other organizations, federal or state officials, media organizations and any other individual or organization in connection with the promotion or publicizing of the activities of ASHP, including advocacy activities.
You hereby grant permission to ASHP and/or ASHP Representatives to use your name, pseudonym or likeness and biographical data in connection with the publication of the Materials. You also understand and agree that you will receive no monetary compensation for participating or for the use of any Materials.
ASHP shall own any copyright and all other intellectual property rights in the Materials. You acknowledge that you have no ownership rights in the Materials. You hereby waive any demand for compensation and waive any claim to any defamation, any moral rights or any violation of rights to privacy, publicity or confidentiality under any statute or common law in connection with any use of the Materials.
You hereby release and discharge any and all actions or claims which you, your family members, your heirs may have against ASHP Representatives and/or ASHP and its officers, directors, employees, contractors, advisors and/or agents, and any other third party contracting with ASHP, arising for any reason whatsoever from any use, publication, distribution, or republication of the Materials in accordance with this Consent and Release and the ASHP Privacy Policy & Policy on Cookies at any time now or in the future. You agree to defend, indemnify, and hold harmless ASHP Representatives and ASHP, and its employees, directors, officers, contractors, advisors and/or agents, from and against any and all claims, demands, expenses, losses or liabilities including, without limitation, reasonable attorney's fees arising out of or in connection with any breach, or alleged breach, of these Consent and Release terms and the ASHP Privacy Policy & Policy on Cookies.
