Developed by the Section Advisory Group on Medication Safety, members discuss Patient Safety Awareness Week and share ideas that organizations can use to bring more awareness to medication safety.
SPEAKERS
Jennifer Matias, PharmD, BCPS, CPPS currently serves as senior manager of pharmacy quality and medication safety at Kaiser Permanente. After completing residency training, Jennifer worked as a pharmacist for 10 years in a variety of hospital settings including community-based hospitals, a pediatric hospital, and the Veterans Affairs Health System. She is a current member of the Medication Safety Advisory Group within the Section of Inpatient Care Practitioners at ASHP.
Michael Claro Dejos, PharmD, MBA, BCPS, CHOP, DPLA, LSSBB is the system medication safety officer at Methodist Le Bonheur Healthcare in Memphis, TN. Mike is a member of ASHP and is past chairman of the Medication Safety Advisory Group. He has served as an expert advisory member for the Institute for Safe Medication Practices for a number of resources and guidelines. His practice interests include system-level error reduction strategies, medication event detection, and change management.
Jeremy Hanner, PharmD, BCPS obtained a Doctor of Pharmacy degree from the University of Arkansas for Medical Sciences College of Pharmacy in 2017. His career began at a rural hospital with clinical staffing and informatics, and today he serves as the Medication Safety Officer for St. Bernards Medical Center. He is a current member of the Medication Safety Advisory Group within the Section of Inpatient Care Practitioners at ASHP.
Elizabeth Wade, PharmD, BCPS, FASHP is the Patient Safety Officer for Amazon Pharmacy. She earned her Doctor of Pharmacy from Northeastern University, and completed a Pharmacy Residency and Medication Use Safety Residency at The Johns Hopkins Hospital. She has served as the Medication Safety Officer for Adult Med/Surg at The Johns Hopkins Hospital, and as the Medication Safety Officer for Capital Region Healthcare in New Hampshire. She works closely with ASHP and ISMP on advocacy for advancing medication safety.
Sarah Stephens, PharmD, CPPS is the network medication safety officer for HonorHealth in Scottsdale, AZ. She earned her Doctor of Pharmacy degree at the University of Wyoming and completed two years of residency training at University of Utah Health specializing in Drug Information. Previous practice includes drug information, acute care adult internal medicine, and academia. Dr. Stephens has held multiple leadership positions within the Section of Inpatient Care Practitioners, currently serving as Chair.
