Person-Centered Medication Safety: A National Quality Forum Action Team

As a special edition Patient Safety Awareness Week podcast, speakers discuss their work on the National Quality Forum’s Action Team on Person-Centered Medication Safety. The co-chairs of this Action Team highlight the priority challenges and recommendations for stakeholders to take away from this work. Additionally, they discuss creating meaningful partnerships with patients, families, and caregivers and outlining practical steps that hospitals and health systems can take to implement better person-centered medication safety.

SPEAKERS

Lisa MorriseLisa Morrise, executive director, Consumers Advancing Patient Safety, advocates for patient - provider collaboration. Morrise consults with numerous organizations about patient safety including NQF, NCQA, CMS, IHI, IPFCC, PFCC Partners and hospital associations. Morrise was named Broadcasting Student of the year and has a MA in Radio-TV-Film. Morrise was a broadcast executive prior to having a child with extreme medical issues. Her concerns around medical error led to her advocacy for patient quality and safety.

Mary Ann KliethermesMary Ann Kliethermes received a BS in pharmacy from the University of Michigan (1977) and Doctor of Pharmacy from the Philadelphia College of Pharmacy and Science (1981). Her 45 year clinical practice includes hospital, home infusion, ambulatory practice and academia. Currently she is Director of Medication Safety and Quality at ASHP. Her expertise is pharmacist reimbursement, medication optimization, safety and quality, and new healthcare models and payment systems.

Rena SackettRena Sackett, PharmD, BCPS, serves as director, Member Relations at ASHP and is the liaison for the Section of Inpatient Care Practitioners. She previously completed ASHP’s Executive Fellowship in Association Leadership and Management and completed a PGY1 Pharmacy Residency at UPMC Western Psychiatric Hospital in Pittsburgh, Pennsylvania. She earned her Doctor of Pharmacy degree from Loma Linda University School of Pharmacy in Loma Linda, CA.
