Amy E. Hyduk-Cardillo, PharmD, MBA, BCPS is a lead surveyor for ASHP. Amy is a graduate of Butler University College of Pharmacy and Health Sciences. She earned an MBA in Healthcare Management from Indiana Wesleyan University. Prior to becoming a Lead Surveyor with ASHP in 2018, Amy worked for 15 years with Lutheran Health Network (LHN) in Fort Wayne, Indiana. Amy was the Clinical Pharmacy Director and PGY-1 Residency Program Director for LHN where she implemented a PGY-1 Pharmacy Residency Program at Lutheran Hospital as well as implemented and expanded multiple inpatient and ambulatory clinical pharmacy services prior to serving as the Network Director of Pharmacy Services for LHN during the last several years of her tenure there. Amy has served on the Board of Directors for the Indiana Pharmacists’ Association (IPA) where she also served as President of IPA and now currently serves on the board for the Pharmacy Education Foundation. She has also served on multiple hospital, local, state, and national committees and councils as well as served as an Indiana Delegate to the ASHP House of Delegates for several years. Additionally, Amy served as a residency program accreditation Guest Surveyor for ASHP Accreditation Services for three years. She is a recipient of the Glen Sperandio Pharmacist of the Year Award from the Indiana Society of Health-System Pharmacists, the Pharmacy Leadership Award from the Indiana Pharmacists’ Alliance, and the Distinguished Young Pharmacist Award. Amy resides in Alanson, Michigan where she works remotely for ASHP.

Stephen M. Ford, Pharm.D., BCOP, BCPS, is Director, Residency Accreditation Services and is responsible for managing the Accreditation Services Office and assists with policy and strategy development for all ASHP accreditation programs and services. Stephen received his B.S. and Pharm.D. degrees from The Philadelphia College of Pharmacy and Science; completed an Oncology Pharmacy Residency at Walter Reed Army Medical Center, Washington, DC; and a Fellowship in Immunization Science and Clinical Vaccinology at the Military Vaccine Agency, Falls Church, VA. Stephen served in the US Army for 28 years, was promoted to the rank of Colonel, and held numerous clinical, operational, and staff positions in Army Pharmacy including Chief Pharmacy Officer, Walter Reed National Military Medical Center, Bethesda, MD. Stephen joined ASHP following his military retirement in 2018.

Katrin S. Fulginiti, B.S. Pharm, M.G.A., is Director, Operations in the Accreditation Services Office at ASHP. She received her B.S. in Pharmacy from the Massachusetts College of Pharmacy and Allied Health Sciences and her Masters in General Administration, with emphasis in Health Care Administration from University College, University of Maryland, College Park. She completed an ASHP accredited pharmacy residency at Children’s Hospital National Medical Center in Washington, D.C. Katrin previously held a position as senior clinical pharmacist at the Washington Hospital Center, Washington, D.C. Prior to joining ASHP in May 2007, she was the Academic Affairs Coordinator for pharmacy services at Kaiser Permanente Mid-Atlantic States Region. Her duties included staff development and training activities, as well as pharmacy student rotation coordination and summer intern placement. She also was the residency program director of the PGY1 pharmacy residency program and the director of the pharmacy technician training program at Kaiser Permanente, both accredited by ASHP. She has held positions within the Accreditation Services Office as an Accreditation Services Associate (lead surveyor), Director of Residency Quality and Improvement, and, Director of Residency Accreditation Services.

Andrea Walsh Haron is an accreditation services associate in ASHP's Accreditation Services Division. Andrea has been employed as ASHP's lead surveyor since January 2015, conducting accreditation site surveys for pharmacy residency programs throughout the United States. Andrea earned her Bachelor of Science in Pharmacy from the Massachusetts College of Pharmacy and Allied Health Sciences in Boston, and her Doctor of Pharmacy from the University of Rhode Island. Prior to joining ASHP, Andrea worked as senior clinical pharmacist specialist at Rhode Island Hospital and The Miriam Hospital, Lifespan affiliates, in Providence, RI for morethan 20 years. She was a preceptor in and coordinator for both PGY2 residency programs, the ASHP-accredited PGY2 Oncology program and the PGY2 Ambulatory Care program, in candidate status. She coordinated accreditation efforts for the residency program and the retail specialty pharmacy program (URAC) and she was involved in maintaining Joint Commission accreditations. Andrea's professional background includes experience in pediatric and pediatric psychiatric pharmacy practice, as well as community acute care hospital settings. Andrea has served as legislative chair and president-elect for the Rhode Island Society of Health-System Pharmacists (RISHP). In 2014, she was also a delegate to the ASHP House of Delegates, representing Rhode Island.

Lisa Lifshin is senior director, pharmacy technician program accreditation and residency services in ASHP’s Accreditation Services Office.She has been with ASHP since 1995 and has conducted close to 1,000 residency and pharmacy technician education and training accreditation surveys.She earned her Bachelor of Science in Pharmacy from the Philadelphia College of Pharmacy and Science and completed her ASHP-accredited hospital residency at Children’s National Medical Center (CNMC) in Washington, D.C. Prior to starting her career at ASHP, Lisa held several clinical and management roles at CNMC including team leader, nutrition support and general medicine; coordinator of the nutrition support team; and supervisor of sterile products. Lisa serves as the Secretary of the Pharmacy Technician Accreditation Commission and is responsible for all areas related to the accreditation of pharmacy technician education and training programs. In addition, she was involved in the development and currently maintains PhORCAS, the online centralized residency application service utilized by all residency applicants and residency program directors. Lisa coordinates the scheduling of onsite residency and pharmacy technician education and training programs for accreditation and still conducts surveys across the country. Prior to her career at ASHP, she held several clinical and managerial positions at CNMC including team leader, nutrition support and general medicine; coordinator of the nutrition support team; supervisor of pharmacy sterile products production. Lisa served on various hospital, local, state, and national committees and boards regarding pharmacy and nutrition support issues.

Mitsi H. Lizer is an accreditation services associate in ASHP's Accreditation Services Division. Mitsi has been employed as a lead surveyor since 2016 and is conducting accreditation site surveys for pharmacy residency programs throughout the United States. Mitsi earned her Doctor of Pharmacy from the University of Michigan College of Pharmacy in Ann Arbor, Mich. She comes to ASHP from the Iowa City VA Health Care System, where she was a clinical pharmacy specialist in mental health from 2014 to 2015. Previously, she worked as professor of pharmacy practice at Shenandoah University School of Pharmacy in Winchester, Va., for more than 15 years. Mitsi was director of an ASHP-accredited PGY2 Psychiatric Residency Program at Shenandoah University/Winchester Medical Center. In addition to clinical and residency responsibilities, she has served on various university, hospital, community, and national committees. Mitsi is a board-certified psychiatric pharmacist and a member of the College of Psychiatric & Neurologic Pharmacists (CPNP) and ASHP.

Dr. Michelle McCarthy is a graduate of Mercer University and completed an ASHP-accredited Drug Information Practice Residency at the University of Pittsburgh. She spent two years as a faculty member at Samford University School of Pharmacy followed by 24 years at University of Virginia (UVa) Health. At UVA Health, she served as the Pharmacy Education and Graduate Programs Coordinator and residency program director for the PGY1-Pharmacy residency (2007-2021) and PGY2-drug information residency (1998-2017) programs. Under her leadership, the UVA PGY1 Pharmacy residency program a recipient of the ASHP Foundation’s Residency Expansion Grant of $40,000 in 2015 and the ASHP Foundation Residency Excellence Program Award in 2017. During her time at UVA, she also served as the residency coordinator for UVA’s 11 other pharmacy residency programs, the Director of the Virginia Commonwealth University School of Pharmacy (VCU SOP)/ UVA Division, and previously managed UVA’s drug information/ medication use policy service, Pharmacy and Therapeutics Committee, infusion pump library and practices, investigational drug service, and the benign hematology and antimicrobial stewardship/ infectious diseases clinical pharmacy teams. Dr. McCarthy’s awards include 1997 Samford University School of Pharmacy Teacher of the Year, 2003 Virginia Society of Health-System Pharmacists (VSHP) Pharmacist of the Year, 2008 Louis P. Jeffrey Preceptor of the Year, and 2015 recipient of the Distinguished Drug Information Practitioner Award by the American College of Clinical Pharmacy DI PRN, 2018 VCU SOP Office of Experiential Education Outstanding Service Award, and 2019 American Association of Colleges of Pharmacy Master Preceptor Recognition. She has been the Education Committee Chair, Continuing Education Administrator, and President of VSHP. She served on the ASHP Commission on Credentialing from 2008-2013 including serving as the Chair in 2012 and participated as a practitioner surveyor for many residency accreditation surveys. From 2018 to 2021, she was a contract lead surveyor with Accreditation Services. In 2021, she transitioned to a full-time Accreditation Services Associate where she works remotely from Charlottesville, VA.

Mary Kate McGinty joined ASHP's Accreditation Services Division in 2015 as an accreditation services associate. As a lead surveyor, she conducts accreditation site surveys throughout the United States. Mary Kate earned her Bachelor of Science in Pharmacy from the Philadelphia College of Pharmacy and Science, and began her professional career as a hospital pharmacist. She practiced at multiple academic medical centers, including Thomas Jefferson University Hospital in Philadelphia, The Ohio State University Hospitals, and Penn State-Milton S. Hershey Medical Center in Hershey, Pa. Throughout her career at University of the Sciences (formerly the Philadelphia College of Pharmacy), Mary Kate held multiple leadership positions in the areas of continuing pharmacy education, accreditation, strategic planning, government and community affairs, and economic development. She also served as adjunct faculty and has a keen interest in teaching. In addition to her degree in pharmacy, Mary Kate also holds a Master of Science in Biomedical Writing.

Michelle L. Rager is an accreditation services associate in ASHP's Accreditation Services Division. Michelle has been employed as a lead surveyor since 2023 and is conducting accreditation site surveys for PGY1 Pharmacy, PGY1 Community-Based Pharmacy and PGY2 specialty pharmacy residency programs throughout the United States. Michelle earned her Doctor of Pharmacy from the University of Maryland School of Pharmacy in Baltimore, Maryland. Following graduation, she stayed at the University of Maryland to complete a PGY1 Community-Based Pharmacy residency with the School of Pharmacy and Happy Harry’s, a Walgreens Pharmacy. She then completed a PGY2 Ambulatory Care Pharmacy residency in Wilkes-Barre, Pennsylvania with the Nesbitt School of Pharmacy at Wilkes University. Previously, she has worked as an associate professor of pharmacy practice and interim department chair at Shenandoah University School of Pharmacy in Winchester, Va., for almost 9 years. During her tenure at Shenandoah, Michelle served as an ambulatory care preceptor for PGY1 residents at Winchester Medical Center and served as the primary preceptor for the Bernard J. Dunn School of Pharmacy, Shenandoah University; Amherst Family Practice & Selma Medical Associates residency program. She worked with the Residency Program Director (RPD) of that program to receive a Pharmacy Residency Expansion Grant from the ASHP Foundation. Michelle later stepped into the role of RPD and served in that role until her departure in 2017. Prior to joining ASHP, Michelle was a Clinical Pharmacist Practitioner working in primary care and the RPD for the PGY1 Community-Based Pharmacy Residency at Novant Health New Hanover Regional Medical Center in Wilmington, NC. She also served as a practitioner surveyor for PGY1 Community-Based Pharmacy residencies for several years. Michelle has been active in the North Carolina Pharmacists Association, serving on the inaugural Executive Board for the Ambulatory Care Forum, and was selected for several ACCP annual committees, as well as, held elected leadership positions within the Endocrine and Metabolism ACCP Practice and Research Networks (PRNs). Michelle is a board-certified pharmacotherapy specialist (BCPS), board-certified ambulatory care pharmacist (BCACP) and a member of ACCP, APhA and ASHP.

Andrea G. Roberson is director, quality and process improvement in ASHP's Accreditation Services Division. She joined ASHP as a lead surveyor in 2012. Prior to her employment at ASHP, Andrea practiced in a hospital setting for more than 20 years. She worked as a clinical specialist, clinical coordinator, clinical manager, and director of pharmacy. Most recently, Andrea was the pharmacy clinical manager at Saint Joseph's Hospital in Atlanta. Her duties included coordinating Pharmacy and Therapeutic Committee activities, serving as program director for the PGY1 residency program, supervising the clinical specialist staff and nutrition support team, and serving as vice-chair of the Institutional Review Board (IRB) and the pharmacy member of the Joint Commission Steering Committee. Andrea holds a Bachelor of Science in Pharmacy and a Doctor of Pharmacy from the University of Georgia. Andrea completed an ASHP-accredited residency at Piedmont Hospital in Atlanta. She served as the Education Committee chair and Governmental Affairs Committee co-chair for the Georgia Society of Health-System Pharmacists (GSHP) and was a GSHP presidential officer from 2004 to 2007. In 2007, Andrea received GSHP's Pharmacist of the Year award.

Renée Acosta earned her Bachelor of Science in pharmacy from the University of Texas at Austin. She practiced in a community pharmacy in Austin as both a staff pharmacist and pharmacy manager. In 1996, she returned to graduate school to pursue her Master of Science in pharmacy through the Option II P. Upon completion of her master's degree in 1998, she became the program coordinator and full-time faculty member for the Pharmacy Technician Program at Austin Community College (ACC). At ACC, Renée developed and maintained the Pharmacy Technician Program curriculum, which included traditional classroom and distance learning classes. In 2003, she received the NISOD Excellence Award for ACC. She has served as an item writer for the Pharmacy Technician Certification Exam (PTCE) and as a member of the Task Analysis Task Force for the Pharmacy Technician Certification Board (PTCB) and the Stakeholders' Policy Council for PTCB. Renée also served a three-year term as a member of ASHP's Commission on Credentialing, and she currently works as a contract lead surveyor for ASHP's Accreditation Services Division.

Marialice Bennett is a professor emeritus of clinical pharmacy and residency director for the Ambulatory and Community Care Residency programs at The Ohio State University College of Pharmacy. She served as a co-founder and pharmacy director of University Health Connection, an innovative interprofessional health care clinic for faculty and staff of the university, from 1999 to 2011. She earned her Bachelor of Science from The Ohio State University College of Pharmacy. Marialice is recognized as a practice innovator. Her contributions to the development of the OSU College of Pharmacy Clinical Partners Program were recognized by the 2004 ASHP Best Practice Award, and her contributions to the development of University Health Connection resulted in the 2005 ASHP Best Practice Award. She received the 1999 APhA Daniel B. Smith Practice Excellence Award and the 2011 OPA Beal Award for contributions to pharmacy. She is an APhA fellow and has been involved with several national practice-based research projects including Project ImPACT: Hyperlipidemia and Patient Self-Management: Diabetes. Marialice's service includes APhA president, president of APhA-Academy of Pharmacy Practice and Management and member of the APhA Board of Trustees (2007-2009), member of the APhA Community Residency Advisory Committee (2002-2007), and member of numerous MTM advisory panels and task forces. Marialice served on the ASHP Commission for Credentialing, representing APhA from 2002 to 2007. Marialice is a contract lead surveyor and will concentrate on PGY1 community pharmacy residency program surveys for the Accreditation Services Division at ASHP.

Jim Carlson is an accreditation services associate in ASHP's Accreditation Services Division. Jim joined ASHP as a lead surveyor in 2016. Jim earned his Bachelor of Pharmacy degree from Washington State University in Pullman, WA and earned his Doctor of Pharmacy from the University of Minnesota in Minneapolis, MN. He comes to ASHP from Regence Blue Cross/Blue Shield in Seattle, where he was Vice President, Clinical Pharmacy Services. Previously, he worked for Group Health Cooperative in Seattle for 30 years in various leadership roles in hospital, ambulatory care and health plan areas. He has received the AMCP Individual Contribution Award and the AMCP Distinguished Service Award. Jim has been the program director of two PGY1 Managed Care Residency programs and been a practitioner surveyor for ASHP since 1999. He has served on the ASHP Commission on Credentialing as the AMCP representative since 2011 and recently completed his term as Chair of the ASHP Commission on Credentialing.

Kristin Eginger, PharmD, BCCCP is a graduate of the University of Florida College of Pharmacy. Following graduation, she completed an ASHP-accredited PGY1 Pharmacy Residency at UF Health Jacksonville in Jacksonville, FL followed by an ASHP-accredited PGY2 Emergency Medicine Pharmacy Residency at CaroMont Regional Medical Center in Gastonia, NC. After the completion of her residency training, Kristin specialized as an Emergency Medicine Clinical Pharmacist and oversaw the PGY2 Emergency Medicine Pharmacy Residency Program as the Residency Program Director for 5 years. In 2017, she was appointed to the Board of Pharmacy Specialties Emergency Medicine Pharmacy Practice Analysis Taskforce to begin the recognition of Emergency Medicine as a specialty practice in pharmacy. She is currently the Pharmacy Operations Manager at CaroMont Regional Medical Center. Prior to joining ASHP as a contract lead surveyor, she served as a practitioner surveyor for PGY2 Emergency Medicine programs for six years and served as a member of the 2018 Emergency Medicine Competency Areas, Goals, and Objectives (CAGO) revision workgroup.

Jeff Hamper is Manager of Academic Relations and Strategic Alliances at Albertsons Companies, which is based out of Boise, Idaho. In this role, he manages affiliations with colleges of pharmacy and oversees the Albertsons PGY1 community-based pharmacy residency programs, intern programs, and experiential programs. Since 2015, he nearly doubled the number of Albertsons community-based residency programs and led the effort to standardize processes for the programs to meet the ASHP and APhA community-based accreditation standards. Jeff earned his BS from Purdue University, his Doctor of Pharmacy from the University of Illinois at Chicago and completed an accredited PGY1 Community-Based Pharmacy Residency in Chicago, IL with Jewel-Osco Pharmacy and Midwestern University. Since completion of his residency, he has been an advocate for the development and expansion of community-based residency programs and has served as a practitioner surveyor for many community-based residency accreditation surveys.

James (Jamie) Kalus, PharmD, FASHP has been a contract lead surveyor with ASHP since 2021. He earned his Doctor of Pharmacy degree at the University of Toledo in Toledo, Ohio and completed a pharmacy residency at the Medical University of South Carolina and a cardiovascular research fellowship at Hartford Hospital and the University of Connecticut. Jamie was faculty at Wayne State University with a practice and research program focused on cardiovascular pharmacotherapy and he has authored several textbook chapters and over 70 articles published in peer-reviewed journals. He is currently a Director of Pharmacy with Henry Ford Health in Detroit, Michigan. Jamie was the PGY1 Residency Program Director at Henry Ford Hospital form 2005 – 2022. He also served on the ASHP Commission on Credentialing from 2014 – 2020, serving as Chair in 2019. Prior to being appointed to the Commission on Credentialing, he served as a practitioner surveyor since 2008. Jamie has also authored articles and provided national presentations related to pharmacy residency training.

Carolyn Kowalchik, RPh, MS is a contract lead surveyor for ASHP. She earned her BS in Pharmacy and Masters in Pharmacy Administration/ Health Services Administration from the University of Utah. She recently retired from Intermountain Healthcare where she was the Director of Pharmacy at Intermountain Medical Center since 2015. Prior to that, she was the Pharmacy Service Director at the University of Utah Hospital where she also was the PGY1 residency program director for 10 years.Carolyn served as a practitioner surveyor for ASHP for many years and recently completed 6 years on the Commission on Credentialing.

Mandy Leonard earned her Bachelor of Science and Doctorate of Pharmacy from Duquesne University. She completed a Drug Information Residency at the Medical University of South Carolina. For the past 24 years, Mandy has overseen the Drug Information/Drug Use Policy Center at Cleveland Clinic. Additionally, she is responsible for coordinating all Cleveland Clinic Residency programs in the health-system. Mandy has helped to start many residency programs at Cleveland Clinic, including Pharmacotherapy and Drug Information (now Medication Use Safety and Policy). She was part of the team awarded the ASHP Residency Excellence Award in 2020. At present, Mandy is the Educational Affairs Director for the Ohio Society of Health-System Pharmacy (OSHP). She is also an Assistant Professor at the Cleveland Clinic Lerner College of Medicine and is the Pharmacology Thread Director. In 2018, Mandy was awarded Pharmacist of the Year by the Ohio College of Clinical Pharmacy (OCCP). Mandy has served as a guest residency practitioner surveyor for the past 8 years and currently works as a contract lead surveyor for ASHP's Accreditation Division.

Matthew Kelm has more than 15 years of experience as a pharmacist, pharmacy administrator, academician, author and researcher. He is the Associate Chief Pharmacy Officer for Inpatient Pharmacy Services and Interim Associate Chief Pharmacy Officer for Ambulatory Care Services at Duke University Hospital in Durham, NC.
Matt’s expertise in human resource management and leadership development contribute to his ability to help clients improve medication distribution and control, operations management, staffing resource needs, regulatory and compliance issues. As a practicing pharmacy leader, Matt possesses extensive knowledge of automation assessment and optimization, use of data to innovate pharmacy and enhance operational processes, facility and clean room technology and design, controlled substances and drug diversion, medication safety, emergency preparedness and innovative models for pharmacy technician education.
Matt earned his Doctor of Pharmacy degree from Purdue University and his Masters in Healthcare Administration from University of North Carolina – Chapel Hill. He is a guest lecturer of the Financial Management of Health-system Pharmacy course for the UNC Eschelman School of Pharmacy Master of Science in Pharmaceutical Science with a specialization in health-systems pharmacy.
He is actively involved in multiple state and national level professional organizations. Dr. Kelm currently serves as the secretary of the Executive Board of the North Carolina Pharmacists Association.

Michael A. Peters, RPh, B.S. BCCCP is a lead surveyor for ASHP. Mike received his B.S from the Wayne State University College of Pharmacy and Allied Health Science in 1987. He worked as a clinical specialist in the Medical intensive care unit at Henry Ford Health main campus in Detroit Michigan. Mike was also the residency program director of the critical care pharmacy residency at Henry Ford Hospital from 2008-2021. In 2020 he transitioned his role from pharmacy specialist to clinical manager. Mike has been an adjunct faculty member with the Wayne state college of pharmacy for 20 years. He joined ASHP as a contracted lead surveyor in the fall of 2021.

Dr. Probst is a Board Certified Pharmacotherapy Specialist with over 35 years of leadership and practice experience in most facets of pharmacy. He spent 20 years at Upstate University Hospital, a tertiary care academic medical center-based health system in Syracuse NY, onboarding as a pediatric pharmacy specialist and advancing in leadership roles including nearly 10 years in Director-level titles. During this time, Dr. Probst started a PGY2 Pediatric Pharmacy Residency and served as its first Program Director, later facilitating the growth of Upstate’s Pharmacy Residencies to include 6 PGY1 positions and 4 different PGY2 programs. Previously, he practiced at other academic medical centers (Yale New Haven Hospital, Medical University of South Carolina), community hospitals, home infusion, nursing home and community pharmacies. Luke recently became a Lead Surveyor with ASHP’s Accreditation Services Division after serving as a guest surveyor for several years. Additionally, as the Managing Partner of LP Rx Advisors LLC, Dr. Probst offers Comprehensive Medication Management and Pharmacogenomics services to patients and providers, and he renders expertise on pharmacology related litigation matters. Dr. Probst has served as Pharmacology course instructor for Nurse Practitioner students, delivered Grand Rounds presentations and didactic lectures to medical and pharmacy students, and he has been an invited presenter at local, state, and national meetings. Luke has co-authored more than 35 articles, a chapter on pediatric pharmacotherapy issues and contributed to numerous professional posters. He serves as a reviewer for several journals, holds faculty appoints in the University of New Mexico’s College of Pharmacy and Upstate Medical University’s Department of Pediatrics, and was Vice-Chair of the Upstate IRB for 15 years. Dr. Probst is a graduate of Albany College of Pharmacy (BS Pharm) and Shenandoah University (PharmD). He has maintained BCPS certification since 1998, is certified in Pharmacogenomics, and held both membership and leadership roles in several pharmacy organizations. Beth Phillips is the Rite Aid Professor at the University of Georgia College of Pharmacy, PGY2 Ambulatory Care Residency Program Director, and Clinical Pharmacy Specialist at the Charlie Norwood VA Medical Center. In addition, she is part of the ASHP Residency Program Design and Conduct Workshop faculty and facilitates workshops at ASHP meetings. Prior to joining ASHP as a contract lead surveyor, she served as a practitioner surveyor for PGY1 and PGY2 programs for 14 years. She received her Doctor of Pharmacy degree at the University of Kansas. She completed a Pharmacy Practice Residency at the University of Illinois at Chicago and a Specialized Residency in Ambulatory Care at the VA Medical Center in Iowa City, Iowa. She is a Board Certified in Pharmacotherapy and Ambulatory Care, and a Fellow of ASHP and ACCP. Prior to joining the University of Georgia, she was a Clinical Pharmacy Specialist in Ambulatory Care at the University of Iowa Hospitals and Clinics and a clinical faculty member at the University of Iowa College of Pharmacy. She has over 20 years of experience as a Residency Program Director and is heavily involved with didactic and experiential teaching of Pharm.D. students, and PGY1 and PGY2 Residents. She has published over 75 journal articles, book chapters, and abstracts, and is currently an Associate Editor for the Journal of the American College of Clinical Pharmacy. She is the past Chair of the ASHP Commission on Credentialing and the 2010 recipient of the Pharmacy Residency Excellence Preceptor Award. Beth currently serves as an ASHP lead contract surveyor of pharmacy residency programs.

Sheri Roumell earned her Bachelor of Science in Management from the University of Mary in North Dakota in 2010. She is a graduate of the Pharmacy Technology program at Casper College and has been a certified pharmacy technician since 2003. Sheri began teaching for the pharmacy technology program at Casper College in 2004 as an adjunct instructor and, in 2006, became the program director and full-time instructor. At Casper College she develops and maintains curriculum and laboratory facilities for on-campus students and for distance-learning classes. She also is responsible for maintaining the accreditation of that program. Sheri has given several presentations on continuing education for both pharmacy technicians and pharmacists. She also presented a proposal to the Wyoming legislature to include a pharmacy technician on the board of pharmacy. Sheri is a contract lead surveyor for ASHP and conducts accreditation surveys for pharmacy technician training programs across the United States.

Suzanne Turner, Pharm.D, FASHP is the Pharmacy Residency and Education Manager and PGY1 Program Director at Lee Health in Ft Myers, FL. She is a graduate of the University of Florida and completed an ASHP-accredited Pharmacy Practice Residency at Methodist Hospitals in Memphis, TN. In her current role at Lee Health, she established the PGY1 program and has oversight for six ASHP accredited programs. While at Lee Health, her duties have also included developing educational, competency and development programs for the pharmacy department; coordinating early and advanced pharmacy student rotations; and serving as the ACPE Administrator. She is an active member of the Florida Society of Health-System Pharmacists and has chaired numerous FSHP councils, including three years as the Education Council Co-Chair. She also served on the FSHP Board of Directors and as the FSHP Chair of the House of Delegates. Suzanne was recognized as the FSHP Pharmacist of the Year in 2012. Prior to joining ASHP as a contract lead surveyor, she served as a practitioner surveyor for PGY1 programs for ten years and served on the ASHP Commission on Credentialing from 2016-2021, including serving as the Chair in 2020. She was a founding member of the Florida Residency Conference Steering Committee and served as the Florida Residency Conference Chair for three years. She is currently serving as part of the ASHP Residency Program Design and Conduct Workshop faculty and serves as an Advisory Board Member for the ASHP Preceptor Competency Assessment Center.

Susan Wong earned her Doctor of Pharmacy from the University of California, San Francisco. She most recently retired from Kaiser Permanente, Santa Rosa as the drug education coordinator and has been actively involved in student and resident training. While the majority of her career was working for Kaiser Permanente, she has also worked at community hospitals such as Petaluma Valley and Sonoma Valley, as well as the Children's Hospital at the Medical University of South Carolina in Charleston. Susan has held academic affiliations at the University of the Pacific, Touro University, University of Michigan, Medical University of South Carolina, Midwestern University, Chicago, and Massachusetts College of Pharmacy. She developed and served as residency program director and preceptor for the PGY1 pharmacy residency program at Kaiser Permanente Marin-Sonoma. She has served in several offices at the local level: North Coast chapter of the California Society of Health System Pharmacists: secretary, treasurer, president, Clinical Affairs, and Continuing Education committees, at the state level: membership, nominations, delegate, and as an ASHP delegate. Susan has been actively engaged with the Western States Conference of Pharmacy Residents as treasurer and member of the board of directors. Sue has participated as a volunteer practitioner surveyor for over four years and is a contract lead surveyor for pharmacy residency program accreditation surveys.

Douglas F. Covey, PharmD, MHA, FCCP is a contract lead surveyor for ASHP. Doug is a graduate of the University of Florida College of Pharmacy. He earned an MHA from the University of South Florida. Prior to becoming a Lead Surveyor with ASHP, Doug was employed for over 30 years by the Veterans’ Health Medical Center in Tampa, Florida. He was the Supervisor for Ambulatory Care Clinical Pharmacy services and PGY-2 Ambulatory Care Residency Program Director. During his tenure at the V.A. he grew the program to over thirty ambulatory clinical specialists in primary care and specialty areas. In addition, for 25 years he was an associate professor, and Assistant Director for the University of Florida’s Working Professional (non-traditional) Pharm.D. program. Doug had served on the Board of Directors for the Florida Society of Health-System Pharmacists (FSHP), member and then Chair of the inaugural BPS Ambulatory Care Council charged with developing certification for ambulatory care specialists (BCACP), and Chair of the ACCP Ambulatory Care PRN group. Doug is well known to ASHP Accreditation Services having been a practitioner surveyor since 1995, served as a member of the Commission on Credentialing (2007-2010), and assisted in developing the initial PGY2 Ambulatory Care residency CAGOs.
