Shannon's Story

Dr. Reidt received her Pharm.D. degree from the University of Minnesota in 2006. She completed a two year Pharmaceutical Care and Leadership residency at the University of Minnesota in 2008. While in residency, she earned a Master's in Public Health with a focus on Public Health Administration and Policy. In 2015, she completed an American Foundation for Pharmaceutical Education Pharmacy Faculty Development Fellowship in Geriatric Pharmacy.

Dr. Reidt currently serves as a member of the Educational Steering Committee for the Section of Ambulatory Care Practitioners.

Facility

Dr. Reidt practices at Hennepin County Medical Center--an urban health system comprised of a teaching hospital and a network of primary care clinics and pharmacies. Dr. Reidt is part of the ambulatory care pharmacy department where she is responsible for two practices. She provides comprehensive medication management to health system patients in their homes. She also provides transitions of care services to health system patients admitted to and discharging from an Augustana Health Care Center, a skilled nursing facility unaffiliated with the health system.

Recent Significant Projects

Dr. White currently serves as the co-chair of the local hospital’s palliative care committee in developing an inpatient palliative care consultation team, a service that has not previously existed in the Most recently, Dr. Reidt has been involved in projects evaluating the impact of ambulatory care services in the health system. Related to home-based comprehensive medication management (CMM) services, she led a team that evaluated the number and type of medication related problems among those patients receiving CMM. The team also analyzed referral sources and follow up rates for home-based CMM patients. Related to transitions of care services for skilled nursing facility (SNF) patients, Dr. Reidt led a team that compared the rate of hospitalizations and emergency department visits 30 days after SNF discharge among those patients who received collaborative transitions of care services and those who received standard of care.

“Use organizations like ASHP to build a network of pharmacists in your specialty area so you have colleagues with whom you can collaborate! Become familiar with literature in your specialty area. I have been successful in developing clinical practices because I keep up to date on literature related to ambulatory care practice development and evaluation. We can learn so much by reviewing the literature!”
