SAFE DISPOSAL OF PATIENTS’ HOME MEDICATIONS
To minimize the patient safety consequences and public health impact of inappropriate disposal of patients’ home medications by working collaboratively with other interested organizations to (1) develop models for patient-oriented medication disposal programs that will minimize accidental poisoning, drug diversion, and potential environmental impact, (2) advocate that the pharmaceutical industry and regulatory bodies support the development and implementation of such models, and (3) educate health professionals, regulatory bodies, and the public regarding safe disposal of unused home medications.
This policy was reviewed in 2025 by the Council on Pharmacy Practice and was found to still be appropriate.
Rationale
Improper disposal of unused or expired medications poses significant risks to patient safety, public health, and the environment. Accidental ingestion, especially among children and older adults, as well as the diversion of controlled substances, are ongoing concerns tied to medications retained in patients’ homes. Disposal of home medications in trash receptacles increases the possibility of accidental poisoning of children and animals and provides inappropriate access to controlled substances that could be used for illicit purposes. Additionally, flushing or discarding medications improperly can contribute to pharmaceutical contamination in water supplies and ecosystems.
Over the past several years, more options have become available for safely disposing and removing unused or expired medications from patient homes. Examples include medication take-back programs, permanent collection receptacles, mail-back programs, and medication deactivation bags. Despite increased patient awareness and availability of these options, standardized, patient-friendly, and affordable disposal programs remain limited. Collaborative efforts among healthcare organizations, regulatory agencies, and the pharmaceutical industry are essential to advance safe, sustainable, and widely adopted disposal solutions.
